The Elder Scrolls IV: Shivering Isles is the first full expansion pack in the Elder Scrolls IV: Oblivion series and was released on March 27, 2007. The PC version of the game is available in a retail-only box and retails for $30, while Xbox 360 users can download it via Xbox Live Marketplace for 2400 Microsoft Points (>$29.99) or buy the GOTY (Game of the Year) Edition. As of Nov. 27, 2007, Shivering Isles is available for PlayStation 3 users.

- The Shivering Isles, also known as the Realm of Madness, home of Sheogorath.
- The gateway to the isles is on an island in Niben Bay, outside of Bravil.
- Players will be able to move between Cyrodiil and the Shivering Isles at their leisure, whether they are level 1 or level 30. There is no requirement for entering the Realm of Madness.
- New cities and settlements on the Isles.
- The Shivering Isles will be roughly 25% the size of Cyrodiil
- The gateway to the isles is on an island in Niben Bay, outside of Bravil.
- Over a dozen new creatures such as "hideous insects, Flesh Atronachs, skeletal Shambles, amphibious Grummites."
- New quests, both a main one and side quests
- On the Shivering Isles there will be 32 new alchemy ingredients.
- New ingredients for alchemy which can do the same things as some of those found in Cyrodiil, in more abundance in the Shivering Isles.
- Craftsmen in the cities of Crucible and Bliss have new armor and weapons made from Madness ore and Amber found throughout the new map to utilize in Cyrodiil.
- A new faction called the Court of Madness.
- 250 new achievement points to unlock (Xbox 360 version, the First Version).

The following is a list Achievements present in the Xbox 360 version. These are the multiple rewards you'll be unlocking while passing through the Realm of Madness.
- Tourist (20 Points) - Enter the Shivering Isles.
- Aspirant (20 Points) - Reach Aspirant Rank in the Court of Madness.
- Citizen (20 Points) - Reach Citizen Rank in the Court of Madness.
- Madman (20 Points) - Reach Madman Rank in the Court of Madness.
- Honored Madman (20 Points) - Get the flame and bring it to the Temple of Arden-Sul
- Duke Dementia (30 Points) - Reach Duke of Dementia Rank in the Court of Madness.
- Duke Mania (30 Points) - Reach Duke of Mania Rank in the Court of Madness.
- Regent (20 Points) - Reach Regent Rank in the Court of Madness.
- Defender (20 points) - Reach Defender of the Realm Rank in the Court of Madness.
- Madgod (50 Points) - Stop the Greymarch.

- A serious bug in the Shivering Isles expansion has the potential to irreparably damage your game if the expansion is installed, even if you are not playing any actual Shivering Isles content.
